ALLEVIA’S SERVICES
Allevia provides the community with NDIS registered supports including:
Capacity Building Supports – Supporting a Participant to connect with the services they need and get the support they want (like case management and case work):
- Level 2: Coordination of Supports.
- Level 3: Specialist Support Coordination
Accommodation and support (the main support a person requires to live in the community):
- Assistance in Shared Living Arrangements – Supported Independent Living (SIL).
- Short Term Accommodation (STA) and Assistance – previously referred to as respite care.
- Medium term accommodation (MTA) support (3-6 months).
- Specialist Disability Accommodation (SDA) – specialist ‘bricks and mortar’.
- Individual Living Options (ILO) – minimum of 6 hours of support per day, 7 days per week) with no overnight support.
Assistance with Social and Community Participation – support provided to assist a participant build their capacity to be active in their community:
- Transport (meeting the cost of transport).
- Skills Development and Training (boosting personal skills).
- In home supports:
- Daily Personal Activities – support provided in the Participant’s home to manage daily personal care needs (toileting, showering, dressing, etc…) and specific health care supports (diabetes management, tracheostomy care, etc…).
- Assistance with household tasks including cooking, cleaning, shopping.
School Leavers Employment Support (SLES):
- A new emerging service model, which uses programs of support designed to assist young adults leaving school to gain pre-employment skills.
Where We Operate
The community we refer to is not limited to specific geographical boundaries, meaning the community refers to the people in a geographical location where we decide to operate:
- Group accommodation is provided in the Inner West, Southwest, and Western Sydney Regions.
- Support Coordination provided across the Sydney Metropolitan region.
- Community supports provided across the southwestern region of Sydney.
- Our head office is in Bankstown NSW.
- Our One Village Community Centre is at 554 Hume Highway, Yagoona NSW 2199.
